Collection of Personal Data in Connection with Application for IHP Sports membership
[In accordance with the Personal Data (Privacy) Ordinance (Cap. 486)]

 目 的 說 明
Statement of Purposes

 

  1. 收集資料的目的Purposes of Collection

經會員會籍申請表填報的個人資料，將由運動及潛能發展研究所(簡稱“本所”)用於下列用途﹕
The personal data gathered by means of the sports membership applications form will be used by the Institute of Human Performance (IHP) for the following purposes:

(a)       處理會員會籍申請的相關事宜: 以及
            carrying out activities relating to the application for sports membership of the IHP, and

(b)      方便本所職員與申請人聯絡，並向申請人發放有關本所的資訊。
facilitating communication among staff of the IHP and the applicant; and distribution of IHP’s information to the applicant.

申請人透過申請表提供個人資料，純屬自願。不過，若申請人未能提供足夠的資料，則本所可能無法處理有關會員會籍的申請。
The provision of personal data by means of application form is voluntary. However, if the applicant fails to provide sufficient information, the IHP may not be able to process the application for sports membership.

 

  1. 獲轉授資料的機構的類別Types of Transferees Receiving the Data

在申請表填報的資料只供本所使用，以達致上文第1段所載的目的。除非申請人同意，所有資料均屬保密及並不會交給本所以外的機構或人士。
The personal data gathered in the application form are in pursuance of the purposes mentioned in paragraph 1 above. Without the consent of the applicant, all data collected in this form will remain strictly confidential and will not be transferred to any organizations or individuals outside the IHP.

 

  1. 查閱個人資料Access to Personal Data

根據《個人資料（私隱）條例》（第486 章）第 18條、第22條及附表1第6原則的規定，申請人有權要求查閱和更改所提供的個人資料。查閱的權利包括有權索取在申請表內所填報的個人資料副本乙份。
Applicants have the right of access to and correction of the personal data as provided in accordance with Sections 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance (Cap. 486). The right of access includes the right to obtain a copy of the personal data gathered in the application form.

 

  1. 查詢Enquiries

如對經本申請表所收集的個人資料有任何疑問，包括要求查閱和更改資料，可向本所提出。
Enquiries concerning the personal data collected by means of this form, including the seeking of access and making of corrections, should be addressed to the IHP:
